Short answer: These options solve different problems. Home care agencies send aides for hands-on help (bathing, meals, companionship), paid privately, by Medicaid, or by long-term care insurance. Papa provides companions mainly through health plans such as Medicare Advantage. Hera provides care management for people with Original Medicare and chronic conditions, billed to Medicare. GUIDE is a Medicare model for people living with dementia and their caregivers, offered through participating providers. HOMEWARD by Cardinal is a local, privately paid planning and coordination service for Erie and Niagara counties that ties the pieces together. Many families combine two or more.

Home care agencies
Licensed agencies provide aides who come to the home. This is the hands-on help many parents need. Medicare covers only part-time, short-term skilled home health when a doctor orders it, not ongoing personal care, so ongoing hours are usually paid privately, by Medicaid for people who qualify, or by long-term care insurance. Papa
Papa matches older adults with “Papa Pals” for companionship and everyday help. It mainly works through health plans: Medicare Advantage and other plans contract with Papa, and Papa Plus adds plan-directed tasks such as helping members complete wellness visits or follow up after a hospital stay. Ask your parent’s health plan whether Papa is a covered benefit.
Hera
Hera offers care management from licensed geriatric social workers and nurses, billed to Original Medicare under Chronic Care Management. Per Hera, a parent qualifies with Original Medicare (not Medicare Advantage), a chronic condition, and residence in a state it serves, which includes New York. Hera says 90% of clients pay $0 out of pocket, with Medigap or Medicaid often covering the Part B coinsurance.
GUIDE dementia programs
GUIDE (Guiding an Improved Dementia Experience) is an eight-year CMS model that began July 1, 2024. Participating Medicare providers offer care navigation, a 24/7 support line, caregiver training, and respite services up to $2,500 a year for eligible patients. Ask your parent’s doctor or neurologist whether a GUIDE program serves Western New York; CMS publishes a participant list.
HOMEWARD by Cardinal
HOMEWARD is a local aging-at-home service for Erie and Niagara counties. We start with a free Snapshot or call, then a $299 in-home assessment and a written plan, with optional monthly coordination. HOMEWARD coordinates; hands-on aide hours still come from a home care agency, family, or public programs, and we can help you line those up. If more care is needed later, we connect families to Cardinal Care Advisors.
Which combination fits?
- Needs hands-on help most days: home care agency hours, plus coordination.
- Has Original Medicare and chronic conditions: check Hera or your doctor’s own care management program.
- Living with dementia: ask about GUIDE.
- Has Medicare Advantage: ask the plan about companion benefits like Papa.
- Family stretched thin or far away: HOMEWARD can quarterback the plan locally.
Common questions
Does Medicare pay for aging in place?
Partly. Original Medicare covers short-term skilled home health and, for eligible people, care management programs like Chronic Care Management and the GUIDE dementia model. It does not cover ongoing personal care aides.
Is Hera available in Buffalo?
Hera says it serves New York State and requires Original Medicare plus a chronic condition. Confirm eligibility with Hera directly.
Can I use HOMEWARD with a home care agency?
Yes. HOMEWARD plans and coordinates care; a home care agency provides the aide hours. Many families use both.
