HOMEWARD is a planning, navigation and coordination service. When a family needs licensed home-care or clinical services, HOMEWARD helps the family identify appropriate independent providers rather than representing itself as the licensed provider.
How does HOMEWARD start?
Start free with the two-minute Snapshot or a 15-minute call. If HOMEWARD is a fit, the next step is the $299 in-home assessment. That visit creates your HOMEWARD Plan, and then your family chooses whether ongoing monthly coordination would help.
